Financial Procedure

It is your responsibility to check to see if we accept assignment on your particular insurance.

We ask for a percentage deposit (co?payment) of what we estimate to be the total cost of the services to be performed. The insurance claim is then submitted to the insurer(s). If and when it is paid, ambulance you will be billed for your share of any unpaid balances according to the plan that you have. Once all insurance payments are in, sickness you may receive a rebate check if there have been any overpayments made by you. Refund checks are sent out once per month from this office. Interest will be charged on all outstanding balances of more than 60 days at the rate of 1.5% per billing cycle.

Please understand that insurance and claim filing can be a very complicated and long process. This office will do the best it can to assure that claims are filed correctly and that the proper benefits are received. If we do not receive payment from your insurance company in 60 days, you will then be billed directly for any outstanding balance. You, the patient, have a lot more influence and legal rights with your insurance company and employer than we do. By getting involved in the payment process on a delayed claim, you will usually get a lot more accomplished than we can alone.



To accommodate the needs and requests of our patients, we have enrolled in numerous medical and dental insurance programs. While we are pleased to provide this service to you, it is very difficult for us to keep track of all the individual requirements of the plans. Each plan has different restrictions requiring how often services may be rendered and, more importantly, where you should obtain these services.

Even within a single insurance company, the plans differ, depending on what type of contract your employer has negotiated.

Providing the highest quality of medical and dental care for our patient is our PRIMARY CONCERN. We are more than willing to provide that care within your insurance guidelines, whenever possible.

Unfortunately, if you do not inform us of special requirements required by your plan and we order treatments that you need, such as lab work, X?rays, hospitalization or services and supplies that are not covered by your plan, we may bill you directly for those charges. Payment for these services may then become your responsibility.

With your cooperation and our help, you should be able to receive all of the benefits you are entitled to, and we will be able to focus our efforts on your medical and/or dental needs.
